For forms and information on  the Sharp HMO plans, please contact our office.  We’ll send the rates, benefits and applications to you via email. Once completed please return applications or change forms to our office for processing no later than the first week of December.

Once an application is submitted  the carriers will not give updates as to timelines and status.  Should they require further information or clarification, you will be notified.  Once approved the carrier will notify both you and our office at the same time.

  • Processing at Sharp can take a few weeks before you are in their system and able to show proof of  insurance to your Doctor, Hospital or Pharmacy.
