Below are links to download the forms and information for the Sharp HMO plans. Once completed please return to our office for processing before 12/11/2020.

Once an application is submitted  the carriers will not give updates as to timelines and status.  Should they require further information or clarification, you will be notified.  Once approved the carrier will notify both you and our office at the same time.

NEW APPLICANTS:  You must pay your premium with your debit/credit card. Visit www.sharphealthplan.com/payment to select your preferred payment type. Alternatively, mail your check or money order prior to the start date to: Sharp Health Plan P.O. Box 57248 Los Angeles, CA 90074-7248 – Be sure to include your Sharp ID from your approval notice. Approval notices may arrive after start date.  If you’re approved but not in the system yet, please call Sharp at 858-499-8300 for assistance in making the payment.

IMPORTANT INFO for CURRENT SHARP CLIENTS: If you pay your premium through Sharp Health Plan’s auto pay program online, you will need to update your account with your new 2021 premium amount before Dec. 25, 2020. This will ensure accurate and timely premium payments and prevent any disruptions in your coverage.

  • Sharp Health Plan will automatically re-enroll you and members of your household into your current health plan, if you do not actively renew your insurance or select a different health insurance plan, if you remain eligible and your plans are still available.
    Even after you have been automatically re-enrolled, you will be able to make changes to your health plan until Dec. 11, 2020, for coverage beginning Jan. 1, 2021
  • You will continue to receive your monthly bill from Sharp Health Plan. Pay your premium directly to Sharp Health Plan by the due date to continue your coverage for Jan. 1, 2021

    Processing at Sharp can take as long as 4 to 6 weeks before you are in their system and able to show proof of  insurance to your Doctor, Hospital or Pharmacy.
